港府於2022/23年度《財政預算案》提出,研究豁免各大專院校醫療相關課程向醫管局繳交臨牀實習費用。醫管局回覆查詢時指,會在今年度豁免院校繳費,而同年度公立醫院為物理治療及職業治療學系學生提供的實習機會及名額安排不變。

不過,有傳局方有意長遠或改職業物理治療系實習模式,職業治療學會及物理治療學會主席均表示,擔心此會實習質素。醫管局回覆查詢時重申,局方會繼續與各持份者溝通,確保臨牀實習的模式及安排可配合培訓及教學需要。

現時實習模式設有臨牀導師專門指導實習生,而相關臨牀導師的職位等同高級職業或物理治療師,有一定的資歷。不同的院校亦會與相關導師分享如何教導學生,導師須確保病人安全之餘,亦要與實習生討論為病人作出什麼的臨牀治療,及就學生的臨牀判斷給意見。

不過,由於豁免各大院校向醫管局繳交實習費,有傳局方有意長遠或改職業物理治療系實習模式,以更好運用現在的資源。職業治療學會主席鄭慧慈承認,醫管局早前與職業治療學會及物理治療學會曾開過會,會上透露未來一年將檢視現行實習模式。

她直言,現時的實習模式臨牀導師要有一定時間的投入,擔心如只簡單改為由全部治療師攤分,會損害實習質素,「唔一定個個都咁鍾意教學生。」物理治療學會會長彭耀宗亦補充,職業及物理治療的實習生須「落手落腳」去做,如臨床判斷錯了治療方案,對病人會有很大影響,故有須要設有臨牀導師專門指導實習生。

他又指,雖然外國指導的實習責任可能會由全部治療師攤分,但外國與本港的工作量不一樣,如本港同事要兼顧日常臨牀工作又指導學生實習的工作,會大大加重工作壓力。他提出,公院佔實習時數約七至八成,另一可行改實習的模式是減少現時在公立醫院的實習時數,改為在非政府機構或地區康健中心等,但他強調,公院實習的經驗對日後成為職業或物理治療師十分重要,並不可以完全取替在公院實習。

醫管局回覆查詢時重申,在2023/24年度新的臨牀實習費用財務安排下,公立醫院為物理治療及職業治療學系學生提供的實習機會及名額安排不變,有關安排亦不會影響現行的臨床實習模式及人手安排。醫管局會繼續與各持份者溝通,確保臨牀實習的模式及安排可配合培訓及教學需要。
