(c) Self-declaration form of physiotherapist’s clinical experience hours (Part 1b)
(d) Declaration before a Commissioner for Oaths, Notary Public, or Justice of the Peace for the two documents (Document (b): Employer’s letter + Document (c): Self-declaration)
